Sometimes tests are administered to gain a better understanding of the severity of someones depression, while at other times, an individual may take the same test several times throughout treatment to track their progress. Most depression tests result in a numerical score. That score provides a mental health treatment provider data about a persons depression. There are many tests that may be used to assess depression. Here are some of the most commonly used. Beck Depression Inventory (BDI-II) The Beck Depression Inventory was created by Aaron Beck, a psychiatrist who is considered to ?be the father of cognitive therapy. The current version of the test, the BDI-II, is appropriate for individuals 13 and over. It contains 21 questions, with each being scored on a scale of 0 to 3. Higher total scores indicate more severe symptoms of depression. Zung Self-Rating Depression Scales (SDS) The Zung Self-Rating Depression Scale assesses the severity of a person’s depression. It consists of 20 items that explore the psychological, somatic, and affective aspects of depression. It’s a quick and simple tool. Major Depression Inventory (MDI) The Major Depression Inventory consists of 10 items and individuals rank how often they’ve experienced certain symptoms over the past two weeks (ranging from never to all the time on a Likert scale). The items are then scored and the score indicates the severity of depression (from no depression to severe). Rome Depression Inventory (RDI) The Rome Depression Inventory consists of a series of 25 items that use the phrases most commonly used by depressed individuals to describe their discomfort. Plutchik-Van Praag Self-Report Depression Scale (PVP) The Plutchik-Van Pragg self-report depression scale uses 34 items to cover the diagnostic criteria for depression. Although it was developed for an earlier edition of the Diagnostic and Statistical Manual (the DSM-III), it remains relevant today as clinicians use the DSM-5. Hamilton Rating Scale for Depression (HRSD) The Hamilton Rating Scale for Depression is a multiple item questionnaire used to identify depression and to evaluate recovery. It’s designed for adults and is used to rate the severity of a person’s depression by evaluating their mood, feelings of guilt, suicidal ideation, anxiety, insomnia, weight loss, and somatic symptoms. Carroll Rating Scale (CRS) The Carroll Rating Scale is a self-report version of the Hamilton Rating Scale for Depression (HAMD). It consists of 52 dichotomous items. Center for Epidemiological Studies Depression (CES-D) The Center for Epidemiological Studies Depression has been used since the 1970s. It consists of 20 items and the self-report version remains widely used. Children’s Depression Inventory (CDI) The Children’s Depression Inventory measures the affective, cognitive, and behavioral signs of depression in children between the ages of 7 and 17. Potential Drawbacks While these tests can be very helpful to the diagnosis and treatment of depression, they do have some potential drawbacks as well.   Self-report inventories are based on someones answers to the questions. The scores can be easily exaggerated or minimized by the person completing the questionnaire.   A person wanting to impress their physician or one who is embarrassed by their symptoms may minimize their symptoms. Similarly, a person who wants to ensure a clinician doesn’t underestimate their suffering may exaggerate how much difficulty they’re experiencing. If a test is administered orally, hearing difficulties or processing disorders may affect a person’s answers. Similarly, a person with poor reading comprehension may not be able to complete a questionnaire accurately. The place in which the test is administered can also impact the answers. If a patient is given a questionnaire to complete in a crowded waiting room or is asked to hand the form to a receptionist, the answers may be different than if the test were given in private or in the presence of a physician only. Additionally, depression impacts the way people think, so individuals who are feeling depressed may not be accurate reporters of their symptoms or they may have poor insight into their symptoms.   What Happens After? If you take a depression test and the scores indicate you may be depressed, a mental health professional is likely going to interview you about your answers and ask more questions to arrive at a clear diagnosis. If you are diagnosed with depression, a mental health professional will discuss treatment options with you. Pros and Cons of the Corn-Based Plastic PLA

Polylactic acid (PLA), a plastic substitute made from fermented plant starch (usually corn) is quickly becoming a popular alternative to traditional petroleum-based plastics. As more and more countries and states follow the lead of China, Ireland, South Africa, Uganda and San Francisco in banning plastic grocery bags responsible for so much so-called â€Å"white pollution† around the world, PLA is poised to play a big role as a viable, biodegradable replacement. Proponents also tout the use of PLA, which is technically â€Å"carbon neutral† in that it comes from renewable, carbon-absorbing plants, as yet another way to reduce our emissions of greenhouse gases in a quickly warming world. PLA also will not emit toxic fumes when incinerated. However, there are still issues with the use of polylactic acid such as its slow rate of biodegradability, its inability to mix with other plastics in recycling, and its high use of genetically modified corn (though arguably the latter could be one of the good effects of PLA as it provides a good reason to alter crop yields with genetic splicing). The Cons of PLA: Biodegradation Rate and Recycling Critics say that PLA is far from a panacea for dealing with the world’s plastic waste problem. For one thing, although PLA does biodegrade, it does so very slowly. According to Elizabeth Royte, writing in Smithsonian, PLA may well break down into its constituent parts (carbon dioxide and water) within three months in a â€Å"controlled composting environment,† that is, an industrial composting facility heated to 140 F and fed a steady diet of digestive microbes. It will take far longer in a compost bin, or in a landfill packed so tightly that no light and little oxygen are available to assist in the process. Indeed, analysts estimate that a PLA bottle could take anywhere from 100 to 1,000 years to decompose in a landfill. Another issue with PLA is that it must be kept separate when recycled, lest it contaminates the recycling stream; since PLA is plant-based, it needs to be disposed of in composting facilities, which points to another problem: There are currently a few hundred industrial-grade composting facilities across the United States. Finally, PLA is typically made of genetically modified corn, at least in the United States. The largest producer of PLA in the world is NatureWorks, a subsidiary of Cargill, which is the world’s largest provider of genetically modified corn seed. This is tricky because the future costs of genetic modification (and the associated pesticides) to the environment and human health are still largely unknown. Pros of PLA Over Plastics: Utility and Biodegradability Genetically modified foods may be a controversial issue, but when it comes to genetically spicing plants together to breed corn that yields more crops for industrial use has its major advantages. With the increasing demand for corn to make ethanol fuel, let alone PLA, it’s no wonder that Cargill and others have been tampering with genes to produce higher yields. At least harmful plastic isnt be used as frequently anymore! Many industries are using PLA because they are capable of biodegrading at a much faster rate than plastic while still offering the same level of sanitation and utility. Everything from plastic clamshells for food take-out to medical products can now be made from PLA, which drastically reduces the carbon footprint of these industries. While PLA has promise as an alternative to conventional plastic once the means of disposal are worked out, consumers might be better served by simply switching to reusable containers, from cloth bags, baskets, and backpacks for grocery shopping to safe, reusable (non-plastic) bottles for beverages.

Trujillo began to work with another politician, Rafael Estrella Ureà ±a, to seize power from Vzquez. On February 23, 1930, Trujillo and Estrella Ureà ±a engineered a coup that eventually resulted in both Vzquez and Alfonseca resigning and ceding power to Estrella Ureà ±a. However, Trujillo had designs on the presidency himself and after months of intimidation and threats of violence toward other political parties, he assumed the presidency with Estrella Ureà ±a as vice president on August 16, 1930. The Trujillo Agenda: Repression, Corruption and Modernization Trujillo proceeded to murder and jail his opponents after the election. He also established a paramilitary force, La 42, designed to persecute his opponents and generally instill fear in the population. He exerted full control over the islands economy, establishing monopolies over salt, meat and rice production. He engaged in blatant corruption and conflicts of interest, forcing Dominicans to buy staple food products distributed by his own companies. By rapidly acquiring wealth, Trujillo was eventually able to push out owners across various sectors, such as insurance and tobacco production, forcing them to sell to him. Vice-President Richard M. In 1936 he changed the name of Santo Domingo to Ciudad Trujillo (Trujillo City) and began to erect monuments and dedicate street names to himself. Despite the vast corruption of Trujillos dictatorship, his fortunes were closely tied to the Dominican economy, and thus the population benefitted as his government went about modernizing the island and undertaking infrastructure and public works projects, such as improving sanitation and paving roads. He was particularly successful in pushing industrialization, creating industrial plants for the production of shoes, beer, tobacco, alcohol, vegetable oil, and other products. Industries enjoyed special treatment, like protection from labor unrest and foreign competition. Sugar was one of Trujillos largest ventures, particularly in the post-war era. Most of the sugar mills were owned by foreign investors, so he set about buying them up with state and personal funds. He used nationalist rhetoric to back up his agenda of taking over foreign-owned sugar mills. At the end of his reign, Trujillos economic empire was unprecedented: he controlled nearly 80% of the countrys industrial production and his firms employed 45% of the active labor force. With 15% of the labor force employed by the state, this meant that 60% of the population depended on him directly for work. Although Trujillo ceded the presidency to his brother in 1952 and 1957 and installed Joaquà ­n Balaguer in 1960, he maintained de facto control over the island until 1961, using his secret police to infiltrate the population and rout out dissent using intimidation, torture, imprisonment, kidnapping and rape of women, and assassination. The Haitian Question One of Trujillos most well-known legacies was his racist attitudes toward Haiti and the Haitian sugarcane laborers who lived near the border. He stoked the historic Dominican prejudice against black Haitians, advocating a deafricanization of the nation and restoration of Catholic values (Knight, 225). Despite his own mixed race identity, and the fact that he himself had a Haitian grandparent, he projected the image of the Dominican Republic as a white, Hispanic society, a myth that persists to this day with bigoted, anti-Haitian legislation being passed as recently as 2013. Trujillo Sr. The LIFE Picture Collection / Getty Images Trujillos anti-Haitian sentiment culminated in the murder of an estimated 20,000 Haitians in October 1937, when he traveled to the border and declared that the Haitian occupation of the border areas would no longer continue. He ordered all Haitians remaining in the area to be murdered on sight. This act provoked widespread condemnation across Latin America and the U.S. After an investigation, the Dominican government paid Haiti $525,000 for damages and injuries occasioned by what officially was termed frontier conflicts. (Moya Pons, 369). Trujillos Downfall and Death Dominican exiles opposed to the Trujillo regime carried out two failed invasions, one in 1949 and one in 1959. However, things shifted in the region once Fidel Castro succeeded in overthrowing Cuban dictator Fulgencio Batista in 1959. In order to help the Dominicans overthrow Trujillo, Castro armed a military expedition in 1959 composed mostly of exiles but also some Cuban military commanders. The uprising failed, but the Cuban government continued urging Dominicans to revolt against Trujillo and this inspired more conspiracies. One widely publicized case was that of the three Mirabal sisters, whose husbands had been jailed for conspiring to overthrow Trujillo. The sisters were assassinated on November 25, 1960, provoking outrage. One of the decisive factors in Trujillos downfall was his attempt to assassinate Venezuelan President Romulo Betancourt in 1960 after discovering that the latter had participated years before in a conspiracy to oust him. When the assassination plot was revealed, the Organization of American States (OAS) severed diplomatic ties with Trujillo and imposed economic sanctions. Moreover, having learned its lesson with Batista in Cuba and recognizing that Trujillos corruption and repression had gone too far, the U.S. government withdrew its longstanding support of the dictator it had helped train. On May 30, 1961 and with the help of the CIA, Trujillos car was ambushed by seven assassins, some of whom were part of his armed forces, and the dictator was killed. 6/5/1961-Ciudad Trujillo, Dominican Republic-Newsmen view the car in which Dominican dictator Rafael Trujillo was assasinated. The song celebrated his death and declared May 30 a day of freedom. Many exiles returned to the island to tell stories of torture and imprisonment, and students marched to demand democratic elections. Juan Bosch, a populist reformer, who had been an early dissident during the Trujillo regime and who had gone into exile in 1937, was democratically elected in December 1962. Unfortunately his socialist-leaning presidency, focused on land reform, was at odds with U.S. interests and lasted less than a year; he was deposed by the military in September 1963. While authoritarian leaders like Joaquà ­n Balaguer have continued to hold power in the Dominican Republic, the country has maintained free and competitive elections and has not returned to the level of repression under the Trujillo dictatorship. As I went through the transcripts of these women I was struck by how casual and nonchalant they were about the business. I can see that because it is legal in the UK these women may not feel the constraints or the fears that some women would feel in other countries. Each interview was unique and one of the women identified herself as bisexual, though she saw many more women clients than she saw men. Since the end of the Cold War, the United States (US) has struggled to define its role as the remaining superpower. Overnight containment and deterrence, the defining strategy of the second half of the 20th century, became questionable and a new grand strategy never emerged to take its place. Without a clear vision, US security policy since the collapse of the Soviet Union has generally relied on maintaining a balance of power in the international system. Then on 11 September 2001 (9/11), the US was attacked by terrorists and suffered its first domestic strategic military shock since the Japanese assault on Pearl Harbor in 1941. In reaction, the US has undergone a historic reorganization of government and is waging an unprecedented global war on terrorism. Yet despite the thousands who have died and trillions of dollars spent since 9/11, the US has not established a coherent, sustainable, and realistic grand strategy that accounts for current circumstances and the future world contex t. In an attempt to fill this grand strategy vacuum and contextualize the current world environment Thomas Barnett has written a series of books that provide a new paradigm for understanding our current and future world circumstance. In the first book, The Pentagons New Map: War and Peace in the Twenty-First Century, Barnett frames the global dynamics in terms of rule sets that govern globalization and provides a vision for achieving global security. In the second book, Blueprint for Action: A Future Worth Creating, Barnett delivers the specifics of actualizing this vision and is the subject of this essay. Specifically, this paper critically analyzes Blueprint for Action because it is inspirational in scope, relevant to the future of US national security strategy, and has implications that warrant serious military consideration. Thomas Barnett is a strategic planner who has worked in national security affairs since the end of the Cold War. Earning a PhD in Political Science at Harvard, Barnett served as professor at the Naval War College, senior advisor in the Office of the Secretary of Defense, and has been involved with several strategic related Department of Defense activities. Over the last decade, Barnett has authored two books, written several articles, and has been widely read by civilian and military audiences. Building off the vision articulated in Pentagons New Map, the thesis of Blueprint for Action is that in order to close the gap between the Core states (healthy and functioning) and the Gap states (unhealthy and disfunctioning) the US must lead a Core state effort to establish the rule sets and institutions that will connect Gap states with the benefits of globalization. This in turn will reduce threats to Core states, promote universal inclusiveness, and foster global peace. Barnett passionately argues, â€Å"we need to make sure our security rule sets match our growing network connectivity, and that our political rule sets keep pace with our economic transactions.† To accomplish this, the book postulates two fundamental actions to make this a reality. First, a System Administrator Force (SysAdmin) comprised of Core state capability must be created to ensure that combat intervention into Gap states have the required follow on forces to win the hearts and minds by rebuilding infrastructure, enabling government, etc. Barnett points out that the US militarys warfighting capacity (Leviathan) and the supporting US economy is not optimized for securing the security environment lying between war and peace. Weapons procurement and service centric force generation designed to support the Cold War high-end strategies of a bipolar world no longer apply to the multipolar globalized environment where low-end fourth generation warfare (4GW) requirements prevail. Given this, the US must â€Å"transform† its military to address existing and forecast security needs and enlist other Core state participation. To this end Barnett posits that dividing the military into a Leviathan force that prosecutes high-end war characterized primarily by US airpower (Air Force and Navy) and a SysAdmin force (Army and Marines) characterized by low-end 4GW proficient capability is required to address the war-peace gap. Moreover, the multinational SysAdmin force would be comprised primarily of other Core states and the significant US contribution would be its global logistics capability and 20% of the overall force structure. This approach would then leverage existing Core state military competencies and create a counterbalance to a US dominated Leviathan force. Even if the White House and all four services bought into this argument, the problem with this construct is that it makes the assumption that Core states will entrust the US with the preponderance of a Leviathan force. Essentially this proposal suggests that non-US Core states command the majority of the soft power and the US command the hard power. Given this, it is doubtful that rising powers such as China will find this arrangement appealing. Similarly, Russia with their resurgence of nationalism along with most European nations that comprise the majority of the Core states will also be suspicious of what would appear to be the US trying to corner the market on high-end warfare. Additionally, Core nations in general will be reluctant to support US combat dominance given its preemptive track record over the last five years. Barnetts blueprint suggests that the employment of force would not happen without the concurrence of â€Å"fellow great powers† because America would be deterred by the realization without the [multinational] SysAdmin little would change in a targeted Gap state. Unfortunately, this logic does not deter the US from pursuing its own vital interests in spite of other Core state objections. What this construct proposes is that the US will monopolize what matters most hard power and global reach. Therefore, the strategy is idealistic in that it does not provide a compelling case for US or its allies. Knowing that a constrained fiscal environment will challenge the preeminence of US power over the next 50 years, rising powers like China will be just as inclined to wait out an eventual decline of American strength. What Barnett fails to provide is a more convincing rationale to support his idea. To do this he needs to address how the US will give up its unilateral preemption policy and assuage Core state fears of US monopolizing hard power. For example, high-end weapon system development could be intentionally spread over multiple Core st ates. By making the US dependent on various foreign sources for arms manufacturing it would leverage the benefits of globalization, increase the interconnectivity of the Core, and create distributed weapon system dependency to keep the US in check militarily. Clearly, many in the US (i.e. military and defense contractors) would object to this, but for a global grand strategy of this nature to succeed checks and balances will need to be structurally imbedded into the institutional framework to foster trust and discourage unilateral tendencies. From a theater strategic joint warfighting perspective, Barnett is proposing a major transformation of US military force structure and strategy that is dependent on other Core state participants as much as they would be dependent on the US. To actualize this blueprint the Army would be optimized for 4GW and the Marine Corps should remain a mini-Leviathan within the SysAdmin force. Additionally, Civil Affairs units embedded in the Special Operations Command (SOCOM) would be integrated into the Army. The idea is to reconfigure US military forces into distinct Leviathan and SysAdmin force structures. This transformation also implies developing the ability to synchronize with the SysAdmin forces of other Core states and provide the logistical framework for rapid global deployment and sustainment. Given the existing issues involved with US force development the scale and scope of developing synchronized Core wide resourcing, doctrine, training, and equipping is questionable. The second postulate that Barnett advances is the requirement to establish global rule sets to guide Core state actions for dealing with politically bankrupt states inside the Gap and individual terrorists. The goal of these rule sets is to create transparency, reduce uncertainty, generate non-zero-sum outcomes, and foster a sense that everyone needs to play by the Cores emerging rules. The problem with the Gap rule set is that initiation of Core action is dependent on the United Nations (UN) Security Council to achieve consensus. Additionally, the rule set does not address how to reconstitute targeted Gap states politically in their transition to peace. Culture, religion, and other factors will clearly make any national rehabilitation unique, and Barnett fails to specify a fundamental political organizing philosophy. Clear political organizing principles need to be articulated to ensure the transparency. Finally, because the blueprint suggests that SysAdmin military force is structu rally divided among Core states the rule set would need to address how Core states will collectively react to an attack. Without a doubt, Blueprint for Action is a â€Å"must read† because it offers a plausible context for understanding the global security environment and a framework for addressing the threats we face today. More importantly, Barnetts paradigm forces readers to leap beyond national constraints and allows for the conceptualization of optimizing civil and military joint capability mulitnationally. Finally, Barnett offers an optimistic â€Å"future worth creating† and plants the seed for an international discussion on proactively securing the future for our planet.
